Membership

Monthly Membership Fee:  

 

$45/month for ages 12-24.

$90/month for ages 25+

Ages 12-17 will need a parent/adult guardian member to register

Monthly fee for weight management = included with monthly DPC membership (cost of medications not included)

​

No long-term contracts, no enrollment fee, no strings attached

​

DOT PHYSICALS : $100 (non-members and members may schedule this using "Patient Appointments" button)
